A 112x form should be completed and appropriate signatures acquired. The form is then forwarded to the Human Subjects Committee Coordinator for approval. Please consult with your department as they may have internal procedures for the submission of Human Subjects protocols.

NO RESEARCH DATA MAY BE COLLECTED FROM HUMAN SUBJECTS UNTIL APPROVAL IS RECEIVED FROM THE HSC.
